Whiplash happens when there is a sudden and impulsive forward-backward movement forced onto the neck (in medical terms, it's called an acceleration-deceleration force).

Whiplash are often caused by motor vehicle accidents on the road such as a car slams into another car's back or side, or similar accidents, and I've even come across freak accidents where people dive into the beach without realizing it's a shallow area, and the force jars the neck into a whiplash injury.

What happens is that the sudden forward-backwards force causes the head to lunge forwards in an almost full neck forward bending, and with a quick backwards snap, the head gets "whipped" back into extension (usually hyper-extension where the head looks upwards).

The damage from the forces generated may cause bony and/or soft tissue injuries in the neck region, such as the articular discs, the ligaments, tendons, muscles and nerve roots, all located in the neck.

Whiplash injuries are graded according to the following:

  • Grade 0: No pain/discomfort, with no physical injury.
  • Grade 1: Mild neck pain, stiffness or tenderness. with no physical injury.
  • Grade 2: Mild-moderate neck pain, stiffness or tenderness; with some physical injury such as tenderness or trouble turning the head.
  • Grade 3: Moderate neck pain, stiffness or tenderness and obvious neurological signs of injury, such as changes to the reflexes or weakness in the arms
  • Grade 4: Severe neck pain and fracture or dislocation of the neck.

What are the symptoms following a whiplash injury?

Most common symptom of whiplash is neck pain and/or stiffness, which can happen immediately following the accident or even after a few days of "feeling nothing".

Other whiplash symptoms may include:

  • Headache
  • Tingling and/or pain in the shoulders and arms
  • Dizziness
  • Altered sensation, pins and needles, numbness in the arms
  • Weakness
  • Visual and auditory symptoms
  • Difficulty concentrating
  • Difficulty in swallowing

Whiplash Physiotherapy: What will be the treatments be?

Physiotherapy management of whiplash injuries will generally be done in a pain-free approach (meaning that you should not feel any or much pain in physio sessions) include:

  1. Exercise as exercise helps to improve pain experienced, range of motion of affected joints, flexibility of the muscles and joints, function and one's fitness. Exercise also promotes production of endorphins, which is a naturally occurring hormone that helps with pain control.
  2. Mobilization of the joints of the spine. It is a gentle and controlled movement of the affected joints which helps to improve mobility in areas of the spine that are limited in motion/movement.
  3. TENS machine (at physio discretion) -  which helps to decrease the level of pain experienced through use of gentle distracting electrical stimulation.
