Tendon Injury, Physiotherapy and Hand Therapy Singapore
A tendon injury can occur when a patient's tendon is exposed to a
strong trauma e.g. a fall, a motor vehicle accident, or as a result of a
pure unprecedented minor accident, such as accidentally hitting one's
finger against the table and sustaining a Mallet Injury (see picture of
mallet injury by Howcheng). A mallet injury is a tendon rupture of the
extensor tendon at the tip and back of a finger, which is common in both
women and men of above 40 years of age.
A tendon is
a collection of strong fibrous connective tissue that when strung
together, becomes a very strong rope-like structure, that connects
muscles to bones, and is very capable of withstanding tension and
strength. They are similar to ligaments, just that ligaments join
bone-to-bone, and tendons joins bones-to-muscles.
have the ability to heal and recover after and injury, though it is
often painful, with a lot of swelling, as well as slow (high risk of
re-injury). After a tendon injury, there is a great need to be attended
to by physiotherapy, occupational therapy and/or hand therapy
intervention, as research has shown that controlled activity and
exercises improves tendon strength, improving overall mobility,
decreasing pain (pain relief) and increasing function and confidence.
a person with a tendinous injury, physiotherapy and hand therapy
intervention varies according to the severity of the injury, the tendon
can be partially lacerated (1-49% torn, termed as mild), moderately
lacerated 50-79% torn, or severely torn 80-95% and total 95-100% torn to
ruptured completely. In the cases of having more than 60% laceration,
patient would require surgery to re-attach the lacerated parts together
to ensure that healing is not compromised, and that the tendon would not
be lengthened. Lengthening of tendons are a complication to tendon
healing, that causes tendons to not be effective anymore (imagine
tendons as tight ropes that pulls an object from one place to another -
too loose would mean that the object, when pulled with the same amount
of force, will cause nothing to happen. Too tight is the opposite, where
there will be pain and limited range of motion).
If the tendon injury is less than 60%, physiotherapy and hand therapy intervention will be: Splinting and Braces to protect the tendon, controlled Exercise and Movement therapy to ensure that the joint, muscle and tendon does not shorten or tighten; Ultrasound to
accelerate the healing process; and when the tendon reaches week 4, we
will start more strengthening activities to further strengthen the
