Pelvic Floor Muscle Tightness and Women's Health Physiotherapy

What is Pelvic Floor Muscle Tightness?

Many patients with pelvic pains have pelvic floor dysfunction, especially having pelvic floor muscle tightness (medically known as hypertonic muscles).

Our pelvic floor muscles are a group of muscles are attach to the front, sides and back of the bottom of our pelvis and sacrum (see picture) - they function much like a hammock, supporting our bladder, uterus, prostate and rectum.

For women, they wrap around your urethra, rectum and vagina.

These muscles must function normally to maintain your ability to pass urine and bowels properly, as well as to support sexual intercourse.

When these muscles are too tight, they will cause pelvic pains, urgency/frequency of your bladder and bowels.

Symptoms of Pelvic Floor Muscle Tightness

Overly tight pelvic floor muscles can cause the following:

  • Urinary frequency, urgency, hesitancy, stopping and starting of the urine stream, painful urination, or incomplete emptying
  • Constipation, straining, pain during or after bowel movements
  • Unexplained pain in your low back, pelvic region, hips, genital area, or rectum
  • Pain during or after intercourse, orgasm, or sexual stimulation
  • Uncoordinated muscle contractions causing the pelvic floor muscles to spasm

Pelvic Floor Muscle Physiotherapy

Pelvic floor muscle conditions and problems are diagnosed by specialist doctors and physiotherapists using both internal and external manual techniques to evaluate the function of these muscles (for your comfort, we only employ external manual techniques in our physiotherapy clinic)

They will assess your ability to actively contract and relax these muscles, and palpate for tight spots or trigger points in these muscles.

We will also physically assess the bones, muscles, joints and soft tissues of your lower back, hips and sacro-iliac joints, as these joint can also strain your pelvic floor muscles.

If your pelvic floor muscles are too tight or weak, they will cause your back and hips to compensate, leading to overuse and to recurrent low back pain and hip pain. It is vital for people who have recurring low back and hip pains who may also have pelvic floor problems (such as incontinence, prolapse, urinary or bowel urgency and frequency, constipation or painful intercourse), to seek a pelvic floor assessment as it may be their pelvic floor problems that is the main cause of their recurrent low back pain or hip pain.

If your pelvic floor muscles are both tight and weak, we will first treat the tightness and tension. Why do this is because often the weakness is caused by the tightness (muscle strength is largely determined by your muscle's ability to contract effectively).

Once your pelvic muscles have reached a normal resting tone, and is able to relax fully, then we will reassess their strength and prescribe pelvic floor strengthening exercises where appropriate.

The range of rehabilitation results vary from patients to patients, depending on a variety of factors and reasons such as severity of condition, genetics, gender and age.
