Bunions are enlargements of the joints at the base of your big toe (medically, it's the metatarsophalangeal joint MTPJ) that occurs when bone or tissue at this joint moves out of their usual place.

This pushes the toes to bend towards the 2nd toe, and causes a painful lump at the bone of the foot.

As this particular joint undergoes most of the body's weight as we walk, bunions can cause serious pain if left untreated. Over time, the MTPJ may become stiff and sore such that wearing footwear can become impossible.

Bunions can also affect the little toe, where it's called a "bunionette"

  • You notice development of a hard/firm nodule or bump at the base of your big toe
  • You find that there is redness, pain, heat and swelling near the base of your big toe
  • You develop corns or other irritations
  • You notice that your big toe and second toe starts to meet and worse, overlap
  • You find that your big toe is not as mobile as before, it's stiffer and less flexible

How Do You Get a Bunion?

Fundamentally, bunions form when the normal forces on joints and tendons of your foot is disrupted, leading to joint instability and even deformity over time. They are usually caused by prolonged abnormal motion and pressure over time (think months and years), which means that they're caused by ineffective foot development and usually caused by the way we walk, our shoes, genetics, and maybe trauma.

Yes, bunions tend to run in families, but it's vital to note that it is not the bunions that is hereditary or passed down, it is that the family generations have bunions because of similar feet type.

Parents who suffer from poor foot mechanics unrealizingly pass their poor foot mechanic issues to their children, who in turn become more prone to having and developing bunions. The abnormal functioning caused by faulty foot development can lead to additional pressure on and inside the foot, leading to bone and joint deformities such as bunions and hammertoes.

Other causes of bunions are traumatic foot injuries, neuromuscular disorders, or conditions that one is born with. Individuals who has flat feet or low arches arches are also at higher risks of developing these problems, as are arthritic patients and patients with inflammatory joint diseases.

Passions and occupations that increases the stress on one's feet also plays a part, such as ballerina and ballet dancers, often develop bunions too.

Wearing shoes that are too tight or shoes that causes the toes to be squeezed together is also a factor.

What Can You Do For Relief?

  • Apply a commercial, non-medicated bunion pad around the bony prominence at the base of your big toe
  • Buy and wear shoes with a wide and deep toe compartment.
  • If your bunion becomes inflamed and painful, apply ice packs several times a day to reduce swelling (anti-inflammatory medication can be helpful to decrease inflammation too)
  • Avoid high-heeled shoes over two inches tall.
  • Consult a podiatrist if the pain persists

Conservative Treatment For Bunion Pain

Depending on the type and severity of your bunions, treatment will differ; although early identifying the problem and potential deformity is the most vital to avoid surgery.

You should consult a podiatrist as early as possible upon experiencing pain or discomfort, because when left untreated, bunions tend to become larger and larger with more and more pain, and more and more tendency to need an operation the longer it is untreated.

The main goal of early intervention is to introduce pressure relief on the bunion and to stop the progression of joint defomity.

A podiatrist may recommend these following treatments:

  • Padding & Taping

    We tend to do this first and as early as possible, as padding the bunion normalizes the pressure and decreases pain quickly, allowing patients to continue a normal and active life. Taping will help to keep the foot in a normal position, reducing stress and pain.

  • Medication

    Anti-inflammatory drugs and cortisone injections are often prescribed to ease the acute pain and inflammations caused by the pressure as well as joint deformities.

  • Physiotherapy

    Physiotherapy is commonly engaged to provide pain relief from the inflammation and the bunion, especially utilizing ultrasound therapy to treat bunions and their associated soft tissue healing involvement.

  • Orthotics

    Customized orthotics and shoe inserts may be useful in controlling foot function and may reduce symptoms and prevent worsening of the deformity.

If early treatment fails or the bunions continues to progress past the benefits of early treatment, you may require podiatric surgery to relieve the pressure and repair the toe joint.

The podiatric physician has a few surgical procedures to choose from, but the main focus of surgery is to remove the bony enlargement, restore normal alignment of the toe joint and to provide pain relief.

One of the most common is a simple bunionectomy, where only the bony prominence is removed, and is used for simple/less severe bunions. Severe conditions may require a more complex procedure, which includes cutting the bone and re-aligning the joint.

Of course, recovery and recuperation will take time, and you will experience discomfort, swelling and pain for a temporary period of time (several weeks post surgery, but it gets better over time)

  • Wear comfortable shoes that conform to the shape of your foot.
  • Wear shoes with a wide and deep toe box.
  • Always fit the larger foot and have your feet sized each time you purchase shoes (sometimes we have feet of different sizes, if you can both different sizes, great, if not, try to buy "up")
