Ankylosing Spondylitis and Physiotherapy Singapore

Ankylosing spondylitis is classified as a form of the arthritis group, a primary character being chronic joint and ligamental inflammation within the spine, which causes pain and decreased range of motion and in many cases, accompanying stiffness. In moderate to severe cases, affected bones within the spine may undergo fusion and join together (called ankylosis) which results in a spine that is very firm and rigid.

As a result, the patient may get an abnormal posture. Other joints that are not so commonly affected includes the joints of the shoulders and neck, the hips, knees and ankles. Sometimes, it also affects the body at the systemic level e.g. affecting organs etc.

What causes this disease exactly scientists and doctors do not know, but what is sure is that in 90% of the cases, has a gene called HLA-B27 - giving rise to a likelihood of genetic factor. But, it's important to note that even if 90% of people with this disease has the HLA-B27 gene, but not all people with HLA-B27 gene has the disease.

Inflammation of this disease often starts at the lower spine and/or at the sacroiliac joints. Early symptoms includes consistent and long term pain and stiffness in the lower back and hip joints. A typical patient will have increased pain or stiffness in the back or hip after a period of immobility or rest.

Symptoms then will progress upwards, towards the spine, possibly affecting the rib cage as well (if this happens, the bones in the chest and ribs may fuse together, and that will result in breathing difficulties as the lungs will not be allowed to expand to intake air).

What we do is to diagnose the presence or absence of ankylosing spondylitis based on symptomatic assessments e.g. presence of pain and stiffness and at which locations; physical assessment e.g. testing for presence of pain and range of motion at the affected area; finding for abnormalities on X-Rays or blood tests on presence of the HLA-B27 gene; and identifying the disease progression if it has migrated, etc. The thing about this disease is that early symptoms can resemble other conditions e.g. osteoarthritis of the hips and low back pain, which are common conditions.

Once it's been diagnosed as ankylosing spondylitis, we will focus on decreasing the amount of pain, inflammation and stiffness experienced in our physiotherapy clinic, using a mix of spinal manipulation and mobilization techniques, soft tissue stretch and management, prevent compensation by strengthening weakened groups, prevent deformity by postural correction and muscle strengthening as well as maintain function and mobility.
