Tarsal Tunnel Syndrome Physiotherapy

Tarsal tunnel syndrome (TTS) refers to the compression to the back of our tibial nerve, and this causes symptoms along the entire path of the nerve from the affected level or inside the ankle or foot.

The tarsal tunnel, like its sister the carpal tunnel, is a very tight and narrow space that is found on the inside of our ankles, and this tunnel is covered with a thick ligament that serves to protect as well as maintain the structures contained in the tunnel (one of these structures is the posterior tibial nerve).

Usually symptoms are usually and typically experienced on the inside and/or on the bottom of our foot. In some patients, symptoms can be isolated and even can happen on one spot.

In some others, it may "radiate/travel" to the arch, toes, heel and even the arch.

Some of the symptoms include:

  • feelings of pain, including experiencing shooting, stabbing pain in and around the ankles and sometimes to/around the toes
  • altered sensations such as tingling, burning, numbness or electricity-like experiences
  • numbness in and around the ankles and toes
  • swelling of the feet and or toes

In some patients, the symptoms come on suddenly and without warning; in some cases the symptoms are brought on or aggravated by overuse of the foot, such as in standing, walking, jogging, running, exercising etc.

Our specialist physiotherapists will first assess and examine your foot to understand, determine and diagnose your foot condition.

There are a variety of treatments that the physiotherapist will prescribe upon determining the causes and core problem, and use these treatments in combination to treat your foot. They are:

  • Immobilization: the "locking" or restriction of the foot and ankle mainly by taping is sometime required to protect the nerve and area and assist the tarsal tunnel and its structure to heal

  • Exercise and specific glide techniques: this is to enhance the nutrient supply to the affected structures as well as improve mobility and motion, reducing symptoms

  • Orthotic devices: we may prescribe our inhouse podiatrist to help or prescibe you directly with corrective insoles that are either customized insoles or off the shelves

  • Shoes: we will also advise you on which shoes are the best fit for your foot for better foot health and recovery

If left untreated, the condition progresses and may result in permanent nerve damage. To add to that, because the symptoms of tarsal tunnel syndrome can be confused with other conditions, proper evaluation by a specialist physiotherapist/podiatrist/foot and ankle doctor is essential so that a correct diagnosis can be made and appropriate treatment initiated.

Tarsal tunnel syndrome is caused by anything that produces compression on the posterior tibial nerve, including

  • flat feet
  • An enlarged or abnormal structure eg. varicose vein,
  • Swelling or inflammation.
